Boarding Forms
All clients will be asked to complete a boarding agreement each time that they drop their pets off to board. This is to ensure that your pets receive the best possible care while they are with us and that we have the most current emergency contact information should the need arise. If you are concerned about time constraints on the day of drop off, or if someone other than you is planning on bringing your pets in, please be sure that you complete these forms prior to check in. Forms are available online and at the front desk.
If your pet requires an exam for diagnostic purposes (ie: skin or ear issues, etc.) or for well health purposes while boarding, please be sure to complete the Exam Request Form in addition to the Boarding Agreement.
Hurricane Season waivers should also be completed if your pet is boarding during the months of June-November.
Medical Requirements
For the safety of all of our patients, we require that all of our boarders be vaccinated (see below) and have had a negative fecal test within the past 6 months. Dogs over the age of two and all cats must be spayed or neutered (unless discussed previously with our veterinarians).
Dogs must be current on their Rabies, DHPP and Bivalent Flu vaccines. Bordetella vaccines must be updated every 6 months.
Cats must be current on their Rabies and FVRCP vaccines.
Isolation accommodations are available (for an additional charge) for pets who cannot be vaccinated in accordance with our policy or for female dogs that are actively in heat.
Flea Prevention
All boarders must also be up to date on flea prevention (applied or administered within the last 30 days). Any pets not current on flea prevention will be treated upon entry at their owner’s expense. You are more than welcome to bring your own for us to give if they will be due for it during their stay.
Belongings & Medications
All belongings from home should be clearly labeled with the pet’s first and last names. T.L.C. Animal Hospital and the Bed & Biscuit will not be held responsible for lost items. Any overly large bedding will not be accepted.
All medications should be supplied by the client and should be in their original packaging with the medical label(s) in place. Please make sure that the “Medication” section on your boarding agreement is filled out completely. The receptionist will verify this information with you at check in and confirm when the next dose is due. Any medications not supplied by the client but written on the boarding agreement will still be given at the owner’s expense.
Peak Boarding/Deposits
Please note that a deposit of 50% of the total estimated boarding charges will be required when making reservations for holidays and Spring Break. 72 or more hours notice of cancellation will be required in order to keep your deposit.
There will be a $10 peak boarding fee for all boarders who are boarding during holidays and Spring Break.
Other
All boarders that require a bath while boarding for hygienic purposes will be charged a $10 fee. This fee is waived for Geriatric boarders.
TLC Animal Hospital will use all reasonable precautions against illness or injury & efforts will be made to contact owners to advise of status. However, procedures and/or treatments, for any abnormality noted, will not be withheld if contact is not made. Payment is required when pet(s) are released or owner returns.
